5.4 Appendixes

APPENDIX 1
APPENDIX TITLE

There should be two blank lines between the appendix title and the first line of appendix material.

Each appendix must have a heading. If there is only one appendix, label it APPENDIX and give it a title. If there is more than one appendix, number them in order: APPENDIX 1, APPENDIX 2, and so on. When inserting a document, chart, image, or other preexisting material as an appendix, add the appendix heading to that material so that it matches the format of the rest of the paper.

Format the first page of each appendix like the first page of a major section, with a two-inch top margin. Place the page number at the bottom center of the first page of each appendix and at the top right corner of any following pages. Appendix page numbers continue the pagination of the paper.

Use the same margins for appendixes as for the rest of the paper. If the appendix contains a scan, image, table, or preexisting document, resize the material as needed so that it fits within the required margins. If the inserted material already has its own page numbers, place those original page numbers in square brackets.
